Upon arrival at camp, parents or legal guardians of minor children will be required to sign a release and submit medical and insurance information for their children. That information is not gathered on this form. If your child will be arriving under care of an adult who is not a parent or legal guardian, please send the completed and signed release and medical information form for your child with the responsible adult. The release can be downloaded here and the medical information form can be downloaded here. The medical information form contains an optional Power of Attorney which will give us the authority to make medical decisions on behalf of your minor child should an incident occur. If you choose not to execute the Power of Attorney portion and get it notarized, your child won't be able to receive medical care until we can contact you. This is enforced by medical professionals here in Missouri.
